What you will need to bring!

The following is needed along with a signed and completed application:

·  Previous 3 months of income verification (For all household members) Income includes:

§   W2, 1099R Income Tax Return, or all or last pay stubs showing year to date

§   award letter or printout from employer ,Social Security Office, Ohio Dept. of Job and Family Services, CSEA, BWC, etc.

·   Most recent utility bills (even if not paid):

§  Primary Heating Bill: Gas/ Propane/ Fuel Oil/ Wood/ Coal/ Electric

§  Electric (if Ohio Edison, both pages)

·  Social Security Cards for All Household Members

·  Driver’s License of individual applying

·  If you are a Homeowner/Mobile Homeowner:

§  Notarized Property Deed or Land Contract

§  Mobile Home Title or Purchase Agreement Property Tax Statement, if available

·  If you are a Renter:

§  Rent receipt and/or Lease Agreement

§  Lot rent receipt Landlord’s Name, Address and Phone Number
